Your photo ID (ages 18 and over)
Your insurance card if you have coverage

Influenza will make its annual appearance this fall and winter, be sure to immunize your family beginning October 15th by visiting our office. Available for everyone 6 months and older. The cost for the flu shot is $25. A small price to pay to avoid the flu. |
